How Do I fill out Scanned Forms?

I don't really know if this belongs to a Computer-As-Source forum because it doesn't have anything to do with audio, but here it goes:

I scanned a form and I want to fill it out on the pc. Is there a program that can help me format a scanned form with textboxes and extract it to a PDF form?

Do you mean that you need a PDF that other people can fill details into? If so, I think that you can "draw" text boxes on to your image with Adobe Acrobat. This is IIRC though because I haven't used Acrobat in a few years.

If you just need to fill in the spaces yourself I would recommend using an image editor. Photoshop works great for this and can save to a PDF.
