Here in the US of A - it has been years since I have seen a BIC Clic (what we used to call the pen on the right). Just great to deal with tests that made me nervous.

Can't help you there, I've always used a fine point sharpie but i never write very small.
Old Parker ball points with new Parker Gel ink cartridges. I just got tired of the fountain pen battles.
Same here! I'm more of a pencil guy most of the time, but when I head out without my schoolbag (and hence an ample supply of writing implements) I usually take a notepad and a Parker Jotter with a gel ink cartridge in it. I like the size and shape of the Jotter, but the ink cartridges they come with are just nasty. The gel ones write wonderfully.
They're also a lot better than any of the (admittedly cheap) fountain pens I have used. I'm not a fan of those at all.

For a while I had a permanent black Staedtler Lumocolor pen that I somehow acquired by accident. It has since disappeared, just like all of my Staedtler pens seem to do, but when I did have it I thought it was just great for writing on CDs because it had an extremely fine tip for a permanent pen. Looking at their website now, I see they actually market a range of Lumocolors specifically for use on CDs and DVDs. I'd definitely suggest giving one of those a try!
