Default 1/4" adaptor stuck in A/V Receiver...help!

Hey everyone, my dad somehow broke the tip of his senn's 1/4" adaptor while it was in the A/V receiver jack...so now it's stuck and the jack can no longer be used. Has this happened to anyone else? Any suggestions on how to get it out?

The same darn thing happened today to my 1/4 adapter for Sony headphone.It was plugged into a LD MKIII and I was very carefull.
here are some pics. I removed the end part which was inside the headphone jack of the amp with a long nose plyer. After removing the power cord!!

Sorry, this camera did not do a good job! It's fired!
