Sennheiser trade in rebate?

I saw this link http://www.head-fi.org/forums/f38/th...t-read-312239/ about trading in old phones to sennheiser and you get money for it. I noticed that the pdf link inside that other link (the form that must be completed) shows a rebate. I want to know if anyone knows exactly how it works. Thanks in advance for any help on this.
